Family Driven Teaming (FDT) is a training and implementation approach that helps youth with complex needs and their families lead coordinated care across systems. Grounded in trauma-informed, strengths-based, and culturally responsive practices, FDT brings together families, natural supports, and providers to create shared, flexible plans focused on what matters most.
Through structured training, consultation, and real-world practice, participants build the skills to partner effectively with families, strengthen support networks, and improve outcomes. Programs can engage at different levels—from foundational training to full implementation—based on their goals and readiness.
